Congratulations, and thank you for booking your trial with Makeup by Kristen! To make the most of our time together, please fill out this form at least 3 days before your appointment. The more detail you give me, the better I can prepare — this helps me pull the right products and plan for your skin before you even sit in the chair.

• Please arrive with a clean, moisturized face (no makeup) unless we've discussed otherwise.

• If you're bringing inspiration photos, feel free to upload them below or send via email/text.

• Let me know about any skin concerns or allergies in advance

• Trials typically run 60–90 minutes.
